Insurance
I am currently a network provider for Blue Cross/Blue Shield PPO health insurance plans. To utilize your insurance please make certain to contact your insurance company to ask about any needs for pre-authorization for treatment and information on your deductible, co-pays and coinsurance coverage. When you make your initial appointment with me, please have the following information available:
* Name of the primary insured on the plan
* Birthdate of primary insured as well as client (if different)
* Group number
* Member/Account number
* Contact number for the plan's Behavioral Health Services and/or general Customer Service
I am considered an out-of-network provider on many other insurance plans. If you would like to use your out-of-network benefits, a receipt will be provided at the time of your appointment with all of the necessary information needed for you to file for reimbursement from your insurance carrier.
If you do have health isurance, please note that services may be covered in full or in part by this policy or your employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- How much does my insurance pay for in-network and out-of-network services?
- Is approval required from my primary care physician?
Reduced Fee
Reduced fee services may available on a limited basis. Please discuss any financial concerns with me in our first session. I do not want payment challenges to keep you from receiving appropriate mental health care.
Payment
Cash, checks and credit cards are accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
